If you were given a chance to write Boruto from the start, how would you have written it? What kind of changes would you have made that differ from the original? How would you lay the plot?

I'd make it Naruto's next generations, where all OG characters remain in the frontline and new gens are getting added along with them. The story would revolve around the mains with upgrading new kids. I'd also develop their stories more and they won't be nerfed (but they will face challenges).
Would give Boruto a different personal and entirely different looks, not a 90% shadow clone of Naruto. Same goes for Shikadai. Similarly, Sarada would be the first green-eyed Uchiha.
Would give most of them better hairstyles, especially Gaara and Naruto.
Their kids shouldn't be aged all same, that's illogical. There will age differences, although they will be in the same age group.
Wouldn't exclude OG characters (eg. Orochimaru, Kakashi, Tsunade, etc) as they will keep playing significant roles.
